Am I eligible to visit IUPUI Campus Health?
All IUPUI students may visit IUPUI Campus Health. See 'Do you accept insurance?' below to see if we accept your insurance, or check with your insurance company.
Estimates of non-emergency health services will be provided within 5 business days of your request. Please send requests to healthfi@iupui.edu.
What forms will I need to complete?
You will be asked to complete an authorization for treatment annually. You may complete the student health history form and bring it with you to save time in the clinic. You will be asked to complete the Patient Communication Authorization form to gain your consent to be emailed or have texts sent to remind you of appointments. The Communication authorization may be revoked at any time by completing the Revocation form.

Do I need an appointment?
Appointments are required. Walk-in appointments may resume after COVID restrictions are lifted. Same day appointments may be available. Please call to schedule an appointment. One of our nurses may triage your concerns to determine the urgency of your concern.
The last appointment at each clinic is an hour before closing.
What if I become ill after hours?
If you become ill or are injured, and you need medical care that cannot wait until our next business day, please visit an emergency room or immediate care facility. See Hospitals, Emergency Rooms, and Urgent Care for locations near campus.
What if I am under age 18?
If you are under age 18 please have your parent or guardian fill out our Minor Consent Form and bring it with you to Campus Health, so we can provide services to you.
How do I submit my immunization information?
I am not a healthcare student.
Immunization information for non-healthcare students is to be submitted to the IUPUI Registrar's office via One.IU.edu.

How do I get copies of my immunization records or other records OR how do I have records sent to IUPUI?
The patient portal is the best way to get copies of records.
Visit your patient portal account to print or email immunizations and labs. Download the Authorization to Release Medical Records form to have records sent to you, another office, or Campus Health. Forms may be faxed to 317-278-6929 or emailed to .
Do I need Tuberculosis Screening or to complete a questionnaire?
The CDC recently changed the tuberculosis (TB) screening requirements. Many hospitals have not changed their requirements.
Healthcare and International students still need annual tuberculosis screening. Screening is done at both clinics.
Students who have a history of positive TB tests need to complete a symptom questionnaire annually. Please download the form, complete it and fax it to Coleman Hall clinic at 317-278-6929 or email it to . You will be contacted if your responses indicate a need for follow-up.
How do I get birth control through the clinic?
An appointment must be made with a physician or nurse practitioner in the clinic for the medication to be prescribed.
If you wish to use the Depo shot, a prescription for the medication will be provided to you to fill at a pharmacy. Bring the medication to the clinic, and a nurse will administer it.
If you have had a pap smear within the last 3 years from a non-campus health provider, please obtain a copy of the result and bring it to your appointment. If you have not had a pap within the last 3 years, please schedule a well-female exam (pap).
The clinic carries a few brands of birth control for uninsured students who qualify for financial assistance: Ortho Tri-Cyclen (generic), Aubra (low estrogen-generic for Alesse, Aviane, and Sronyx), Reclipsen (generic and as close to Yasmin as possible), Emoquette, and the Depo shot.
The Nexplanon implant is available for long-term contraception. IUDs are not currently inserted, but check back for an update.
Uninsured students can also get generic OrthoCyclen and Tri-Cyclen at Walmart, Target, and Kroger for a low cost. Trinessa and Sprintec are available at Kroger. Tri-Sprintec and Sprintec are available at Walmart for a low cost.
Which insurances do you accept?
If the clinic is in-network, IUPUI Campus Health will file insurance claims for students with:

Check with your insurance. Each insurer has many plans. Make sure your plan covers services at IUPUI Campus Health. Co-pays specified by your plan apply to clinic visits.
Uninsured students or with out of network insurance should contact the clinic for financial assistance. A walk-out statement will be provided for students to file with their insurance for reimbursement upon request.

Some universities require students to pay a student health fee with their tuition. Does IUPUI?
Many universities charge a mandatory student health fee to lower the costs of student health visits or to provide many free visits to each student during a semester. IUPUI charges a general services fee. Student Affairs shares a small portion of that fee with Student Health.

What services are available at IUPUI Campus Health?
IUPUI Campus Health provides primary and acute care services to IUPUI students and spouses (if covered by the student's Anthem plan.). For a complete listing of services, please visit Student Health.
Will I be notified if my student visits IUPUI Campus Health for any reason?
Routine visits
If your student visits IUPUI Campus Health under routine circumstances, your student's confidentiality will be maintained, and you will not be notified. IUPUI students under 18 need written parental consent.
Emergencies or urgent visits
Should your student visit IUPUI Campus Health in an emergency situation, and you are listed as your student's emergency contact, you will be notified. Students under the age of 18 will be seen for emergent or urgent care without parental consent. Efforts to contact a parent by telephone will be made in urgent or emergent situations.
How much does a visit cost?
The cost of your visit will depend on the services you receive. Lab and X-ray charges are billed by the companies that provide the service, i.e. IU Health, Labcorp or The Medical Foundation.

My student has called and said they are sick and does not have money to visit IUPUI Campus Health. What can I do?
If your student is sick and needs to be seen, we will see them. You can deposit money onto your student's CrimsonCard or a credit card number can be called to the clinic. Visit the CrimsonCard website for more information. Clinic charges may be billed to the student's Bursar account.
What happens if my student becomes ill or is injured when IUPUI Campus Health is closed?
If your student lives on campus or near downtown, they can visit one of many hospital emergency rooms on campus or in the area.
Does IUPUI require that my child has had any particular vaccinations?
Many universities require students living on campus to show proof of immunity or vaccination against several diseases. IUPUI requires proof of immunization against:
- Measles, mumps and rubella
- Meningitis
- Tdap
- Varicella
- International students need Tuberculosis surveillance
- Influenza - if the student will be on campus
Proof of immunity includes lab tests that demonstrate acceptable antibody levels to the illnesses and/or immunization documentation. Non-healthcare students submit the information to the Registrar's Office. Visit the Registrar's Office for information about the requirements and where to submit your information.
IUPUI clinical programs (medicine, dentistry, nursing, occupation and physical therapy, physican assistant, etc.) determine vaccination guidelines for students admitted to their programs. An additional requirement is for Hepatitis B vaccination (3 doses). Proof of immunization/immunity for healthcare students is submitted to IUPUI Campus Health.
How do I schedule biometric screening appointments on-line?
On-line scheduling is not available at this time.
Do I need Tuberculosis Screening or to complete a questionairre?
The CDC recently changed tuberculosis (TB) screening requirements. IU Residents need to be screened annually either with a blood test, skin test or if skin/blood tests have been positive in the past, by completing a questionairre. IU & IU Health employees do not need annual screening.

I am ill or have been injured, but it is not work-related. Can I come to IUPUI Campus Health for treatment?
IUPUI Campus Health provides occupational health services to employees. The IU Staff & Faculty clinic provides non-work related acute and primary healthcare for employees. If an IU employee’s supervisor is concerned that an employee may be contagious, the employee may be evaluated by Campus Health staff. The employee will be referred to their own primary care provider for treatment. Employees who are an IUPUI student, may be seen and treated at the Campus Center clinic or Coleman Hall clinic for personal health conditions.
Do I need an appointment?
While COVID remains a concern, visits will be in-person or via telehealth.
An initial visit for a work-related injury is done in-person, on a walk-in basis.
Follow-up appointments are made before the employee leaves the clinic.
What should I do if I am injured at work?
Notify your supervisor immediately. An Indiana University incident report or IU Health Accident/Exposure Report (dependent upon if you are employed by IUPUI or IU Health) must be completed. It can be completed on line or on a paper form. Paper forms should be signed by your supervisor and brought to IUPUI Campus Health at Coleman Hall.
Please be certain that the following areas on the IUPUI form are completed: patient name, patient medical record number, account number, payroll clerk name and telephone number, wage rate, employment date with IUPUI, employment date at this job. In addition, please include a detailed description of the incident, along with the names and telephone numbers of anyone who witnessed the incident. Your supervisor must sign the form.
An initial evaluation visit is covered by the entity's Worker's Compensation insurance. Future visits will be covered upon approval by the Worker's Compensation office. Your personal insurance will be billed for visits not covered by Worker's Compensation.
