A Contraceptive Renewal form for those with a current pap result, may be downloaded, completed ahead of time, and brought to the visit or completed at the time of the visit. Student Injuries

Students who sustain injuries that are attributed as being the responsibility of the university may be seen for an initial visit at IUPUI Health Services. Injuries that might be the responsibility of the university include those sustained during class activities, during university-sponsored and supervised field trips, and during time on university property (acting as a student) where injury is caused by some aspect of university property (such as a falling paper towel dispenser). Responsibility is determined by the Indiana University Office of Risk Management. If an injury is sustained, a supervisor (professor, instructor, or other teaching authority with supervisory responsibility over the student or dean or other administrative official with supervisory responsibility over the student) must fill out the Indiana University Risk Management Incident Reporting Form as soon as possible.

For serious or life-threatening injuries, call 911. For minor injuries, the student should be taken to IUPUI Health Services during business hours or to an emergency room or urgent care center when the clinic is closed. The student will be seen by a provider and given the appropriate initial care. The student will be referred to his or her primary care provider for follow-up care. If the injury is determined by Risk Management to be the responsibility of the university, the student will not have to pay for services rendered at the initial student injury visit to IUPUI Health Services. If the Risk Management office does not accept liability for the injury, the student will be billed for services rendered at IUPUI Health Services.

Student Employees

Students employed at IU or Clarian may be seen for work-related injuries. See Employee Health Services for more information.
