IUPUI Health Services is committed to providing quality primary care to students and occupational services to employees. We are here to support your physical health as one component of overall wellness. The general services fee funds the Campus Center Student Health clinic and enables student discounts including free flu vaccines.

Immunizations

Students wishing to know about required immunizations should visit " Immunizations" under Services.

Students living in IUPUI housing need to provide documentation of basic vaccinations (or immunity) before the fall semester begins via this form. The form is provided in your housing packet also.

Medical records

If you need copies of immunizations given at IUPUI Health Services, please visit Student Services or Forms. Please download the Release of Information Form and bring or fax it to the clinic.

If you have questions or concerns, not addressed on the website, please call IUPUI Health Services at 274-8214 or e-mail us at nurse1@iupui.edu. This link is not to be used to request appointments or immunization records.

Appointments and cancellations may not be made via e-mail. You must call IUPUI Health Services at 274-8214 to make or cancel an appointment.
